Proposed Work Plan <br />The goal of the Santa Ana Youth Service Provider program is to deliver a Workforce Innovation & <br />Opportunity Act (WIDA) youth program for Santa Ana youth between the ages of 16-24 that are out-of- <br />school with a focus on providing paid work experience and/or WIDA defined credentials. In addition to our <br />roles and responsibilities as a member of the Santa Ana Youth Service Provider Network, our plan provides <br />full wrap-around support, including basic needs, housing support, mentoring, and work experience or <br />occupational training, for 20 qualified youth over the contract period, July 1, 2025 through June 30, 2026. <br />The SUFK OC Work Plan will meet and exceed the stated requirements and performance outcomes in the <br />SA VSP Scope of Work below: <br />1) Participate actively in the Santa Ana Youth Council and Youth Service Provider Network. <br />2) Attend monthly Youth Service Provider Network meetings. <br />3) Deliver two or more of the 14 WIDA program elements. <br />4) Create and sustain partnerships with businesses, parents and other resources to serve youth. <br />5) Design programs to meet or exceed WIDA Performance Indicators (refer to page 26 for Performance <br />Indicators). <br />6) Recruit and refer potential WIDA participants to the Service Navigator for eligibility determination. <br />7) Enroll 100% enrollment by second quarter (December 31, 2025). <br />8) Register youth participants into CalJOBS. <br />9) Document on CalJOBs (www.caljobs.ca.gov) at least one or more substantial service activity (refer <br />Workforce Services Directives WSD 20-10 and WSD 19-06) each month for each enrolled youth. <br />10) Conduct an objective assessment of the academic levels, skill levels, and service needs of the <br />participant. <br />11) Maintain files that clearly document WIDA eligibility, assessments, needs and services provided to <br />participant and performance outcome verification. <br />12) Provide 12-month follow-up services for "exited" (youth no longer enrolled in program) participants. <br />13) Develop systems to invoice for services on a monthly basis. <br />14) Make available satisfaction surveys to all participants. <br />15) Be prepared to be monitored for WIDA compliance by Federal, State, SAW DB and/or Service Navigator. <br />Approach <br />Our approach applies the SUFK OC standard operating procedures for Community Outreach, Street <br />Outreach and the "Journey to Self-Sufficiency" (JTS) youth mentoring program. For over 20 years, SUFK QC <br />has operated these programs to reach homeless and at-risk youth. During that time, thousands of OC youth <br />have engaged in the SUFK OC mentoring programs and successfully moved from crisis to self-sufficiency. <br />We offer two primary mentoring programs. The goal of "Journey to Self-Sufficiency" (JTS), program is for <br />each youth to move from surviving to thriving. The program typically takes 6 months to complete. At that <br />point, the youth is self-sufficient and permanently housed. Success is measured by the youth remaining <br />self-sufficient and stably housed at 1 year. In 2024, we experienced a 97% success rate with 343 youth <br />enrolled. JTS is a five-stage, goal-based, process that includes activities, workshops, training, work <br />experience and occupational training that build job skills and life skills leading to long-term independence. <br />Youth receive support from their SUFK OC mentor and case manager throughout the duration of the <br />program. <br />Youth under 16 and youth that can be reconnected to secondary education, align with the SUFK QC <br />"Roadmap to Success" (RMTS) youth mentoring program.
