6'k <br /> City Net <br /> Scope of Work <br /> Substance Abuse Coordination Services <br /> Program Overview:City Net will engage with local detox,treatment, and sober living <br /> programs to facilitate a Memorandum of Understanding (MOU) to refer clients with ties to <br /> the city of Santa Ana to substance abuse programs.The SMART program will provide a team <br /> of two drug and alcohol counselors to engage and assist clients experiencing substance <br /> abuse disorders to detox,treatment, and long-term housing to support the client on their <br /> journey to sobriety. <br /> Outreach and Engagement:City Net will assign two certified drug and alcohol counselor <br /> case managers (DACCMs)to engage with clients who are struggling with substance abuse. <br /> Outreach and engagement efforts focus on attending to the clients' immediate needs of <br /> safety,food, hydration to build trust and rapport and a foundation with the client.Once trust <br /> is established,the DACCMs will work collaboratively with the client to create a treatment plan <br /> that will support the client through detox,treatment, and housing. <br /> Drug and Alcohol Assessment: DACCMs will identify problematic use of substances, reduce <br /> excessive use of substances, and determine the severity of the substance use disorder, <br /> physical health status and any co-occurring mental health conditions that may be affecting <br /> the client through the Screening Brief Intervention and Referral to Treatment (SBIRT) <br /> assessment.The SBIRT Assessment is an evidence-based tool that screens for all types of <br /> substance use, not just substance dependence,and provides an individualized treatment <br /> plan addressing the unique needs of each client. <br /> 1. Screening: Universal screening using a brief,validated questionnaire to determine the use <br /> and severity of alcohol, illicit drug and prescription drug use to inform the level of <br /> appropriate intervention. No blood or urine test is administered. <br /> 2. Brief Intervention: Brief motivational and awareness-raising intervention given to those <br /> who substance use is putting their health and well-being at risk.This can take between 5- <br /> 30 minutes depending on the level of client engagement and is provided onsite following <br /> the screening. <br /> 3. Referral to Treatment: Referral to specialty care for clients with high-risk substance use <br /> and/or a possible substance use disorder.The DACCM assists clients with accessing <br /> specialized treatment,selecting a treatment facility, and obtaining insurance <br /> authorizations when necessary.After a referral is made,follow-up phone calls with the <br /> patient or treatment staff are part of the collaboration to ensure care. <br /> Clients are also asked about medical insurance and DACCMs verify their enrollment status <br /> with the Managed Care Plan Provider. MCP enrollment will qualify clients for future housing <br /> services and placement in Drug and Alcohol treatment programs.
