F.XAI<RIT F. <br />FORM OF NOTICE OF AFFORDABILITY RESTRICTIONS ON TRANSFER OF <br />PROPERTY <br />RECORDING REQUESTED BY, <br />AND WHEN RECORDED MAIL TO: <br />City of Santa Ana <br />20 Civic Center Plaza, 6th Floor <br />Santa Ana, California 92702 <br />Attn: Executive Director <br />This document is exempt from <br />payment of a recording fee <br />pursuant to Government Code <br />Sections 27383 and 6103. <br />NOTICE OF AFFORDABILITY RESTRICTIONS ON <br />TRANSFER OF PROPERTY <br />This Notice of Affordability Restrictions on Transfer of Property (or "Notice <br />of Affordability Restrictions") is executed and recorded pursuant to Section 65915 <br />of the California Government Code, and affects that certain real property generally <br />located at 2130 East Fourth Street, Santa Ana, California (APN 400-091-023) <br />(hereafter, the "Property") as legally described in Exhibit A hereto ("Property"). The <br />City of Santa Ana, a charter city and municipal corporation of the State of California <br />(referred to hereafter as "City"), and Olive Crest, a California nonprofit <br />corporation ("Developer/Property Owner") have entered into that certain Density <br />Bonus Housing Agreement With Declaration of Covenants, Conditions and <br />Restrictions, dated as of , 2025 ("Density Bonus Housing Agreement"). <br />1. The Density Bonus Housing Agreement provides for affordability <br />restrictions and restrictions on the transfer of the Property, as more particularly set <br />forth in the Density Bonus Housing Agreement. A copy of the Density Bonus <br />Housing Agreement is on file with City as a public record and is deemed <br />incorporated herein.
