APPENDIX B- GOVERNING BODY RESOLUTION <br />INSTRUCTIONS <br />Purpose The purpose of the Governing Body Resolution is to appoint individuals to act on <br />behalf of the governing body and the applicant. <br />Authorized The Governing Body Resolution allows for the appointment of individuals or <br />Agent(s) positions. For each person or position appointed by the governing body, submit the <br />following information, with the resolution, to the state on the applicant's letterhead: <br />? Jurisdiction <br />? Grant Program <br />? Name <br />? Title <br />? Address <br />? City <br />? Zip Code <br />? Telephone <br />? Fax # <br />? Cell Phone # <br />? E-Mail Address <br />Authorized - If the Governing Body Resolution indentified Authorized Agents by <br />Agent position and/or title, changes can be made by submitting new <br />Changes Authorized Agent information to the State. <br />- If the Governing Body Resolution identifies the Authorized Agent by name, a <br />new Resolution is needed when any changes are made.
