temporary disability compensation payable under the aforementioned Act, <br /> continue to be paid his or her normal salary and accrue other benefits in <br /> accordance with the provisions of Labor Code Section 4850. <br /> B. Any period of time during which an employee is required <br /> to be absent from his or her position by reason of an industrial injury <br /> or industrial illness for which he or she is entitled to receive <br /> compensation shall not be considered a break in continuous service for <br /> the purpose of his or her right to salary adjustment or to the accrual of <br /> vacation and seniority. <br /> ARTICLE XII - EMPLOYEE INSURANCE <br /> Section 1. Effective January 1, 1989, the City will continue to <br /> contribute the following amounts toward the payment of premium for <br /> affected employees and their dependents under health, dental and long <br /> term disability insurance plans provided by the association for employees <br /> covered by this Agreement: <br /> A. Health Insurance. <br /> 1. With respect to each affected employee who does not <br /> have dependents included under a health insurance plan, the City will <br /> contribute up to a maximum of one hundred thirty-three dollars and <br /> thirty-two cents ($133.32) per month based on premium rates in effect <br /> during December 1988 for the plan selected by the employee on or after <br /> January 1, 1989. <br /> If, on or after October 1, 1988, the total monthly <br /> premium for the most expensive "employee only" coverage among the City's <br /> HMO plans exceeds $133.32 per month, the City will increase its <br /> contribution up to an amount equal to $133.32 plus 50% of the amount of <br /> said premium in excess of $133.32 per month. <br /> 2. With respect to each affected employee who does have <br /> dependents included under a health insurance plan, the City will <br /> contribute up to a maximum of two hundred fifty five dollars ($255.00) <br /> per month based on premium rates in effect during December 1988 for the <br /> plan selected by the employee on or after January 1, 1989. <br /> If, on or after October 1, 1988, the total monthly <br /> premium for the most expensive "family coverage" among the City's HMO <br /> plans exceeds $255.00 per month, the City will increase its contribution <br /> up to an amount equal to $255.00 plus 50% of the amount of said premium <br /> in excess of $255.00. <br /> 3. Any contribution necessary to maintain benefits <br /> under said health plans in excess of the amounts set forth above shall be <br /> borne entirely by the Association and/or the enrollees. <br /> -26- <br />
