employees reappointed to the service of the City in accordance with Section <br /> 9-114 of the Civil Service Rules and Regulations, the City will contribute <br /> toward the payment of "family" coverage health insurance premiums in the <br /> same manner as provided in Subsection 1B above, as soon as such employees <br /> are eligible for such coverage after the date of their <br /> appointment/reappointment. <br /> D. Any contribution necessary to maintain benefits under any <br /> health insurance program provided by the City for its employees and their <br /> eligible dependents in excess of the amounts of the City contribution <br /> specified above shall be borne entirely by the employee. <br /> Section 2. Dental Insurance. The City agrees to contribute up to a <br /> maximum of twenty dollars ($20.00) per month per employee toward the <br /> payment of premiums for dental insurance plans provided by the City for <br /> employees covered by this Agreement and their eligible dependents. <br /> Any contribution necessary to maintain benefits under said dental <br /> plans in excess of the amount set forth above shall be borne entirely by <br /> the employees. <br /> Section 3. Long-Term Disability (L.T.D.) Insurance. The City shall <br /> continue to contribute an amount toward the payment of premiums for each <br /> employee who is enrolled in the long-time disability insurance plan <br /> provided by the Association for employees covered by this Agreement in <br /> accordance with the following schedule: <br /> A. With respect to sworn, "safety-member" police personnel, up <br /> to a maximum of forty-eight dollars ($48.00) per month per enrollee. <br /> B. With respect to non-sworn, "miscellaneous-members" personnel, <br /> up to a maximum of forty-four dollars ($44.00) per month per enrollee. <br /> Any contribution necessary to maintain benefits under such long-term <br /> disability insurance plan provided by the Association for covered employees <br /> in excess of the amounts set forth as above shall be borne by the <br /> Association and/or the enrollees. <br /> Section 4. Life Insurance. The City shall maintain in effect for the <br /> term of this Agreement its existing life insurance plans covering employees <br /> covered by the Agreement on the same basis as its plans are offered to <br /> employees as of June 30, 1985. <br /> Section 5. The City shall retain the right to change health, dental, <br /> long-term disability and life insurance carriers, administer the insurance <br /> benefits provided thereunder, and select and/or change any excess or <br /> supplemental insurance carriers as a part of any self-insurance plan during <br /> the term of this Agreement, provided that employees covered by this <br /> Agreement continue to receive the same or equivalent benefits. <br /> Section 6. Optional PBA Benefit Plans. By majority vote, Association <br /> members may opt to receive cash equivalent of premium contributions made by <br /> 26 <br />
