(1) For employees who have less than one (1) year of full-time <br /> service, the amount of City contribution toward "employee only" coverage <br /> specified in "A" above, but no amount for dependent coverage. Such employees <br /> shall have the option to purchase dependent coverage under the plans provided <br /> by the City for its employees by personally paying the amount of difference <br /> between "employee-only" coverage and "family" coverage. <br /> (2) After the completion of one (1 ) year of full-time service, <br /> the City shall contribute toward the payment of premiums for health insurance <br /> in the same manner as provided for in Subsections 1A and 1B, above. <br /> (3) With respect to "lateral transfer" police officers and <br /> employees reappointed to the service of the City in accordance with Section <br /> 9-114 of the Civil Service Rules and Regulations, the City will contribute <br /> toward the payment of "family" coverage health insurance premiums in the same <br /> manner as provided in Subsection 1B above, as soon as such employees are <br /> eligible for such coverage after the date of their appointment/reappointment. <br /> D. Any contribution necessary to maintain benefits under any health <br /> insurance program provided by the City for its employees and their eligible <br /> dependents in excess of the amounts of the City contribution specified above <br /> shall be borne entirely by the employee. <br /> Section 2. Dental Insurance. Effective November 1 , 1983, the City agrees <br /> to contribute up to a miximum of twenty dollars ($20.00) per month per <br /> employee toward the payment of premiums for dental insurance plans provided by <br /> the City for employees covered by this Agreement and their eligible <br /> dependents. <br /> Any contribution necessary to maintain benefits under said dental <br /> plans in excess of the amount set forth above shall be borne entirely by the <br /> employees. <br /> Section S. Long-Term Disability (L.T.D.) Insurance. Effective November <br /> 1, 1983, the City shall contribute the following amounts toward the payment of <br /> premiums for employees covered by this Agreement under the existing long-term <br /> disability plan maintained by the City for its employees: <br /> A. With respect to sworn, "safety-member" police personnel, up to <br /> a maximum of twenty-seven dollars ($27.00) per month per employee through June <br /> 30, 1984; and up to a maximum of forty-eight dollars ($48.00) per month per <br /> employee effective July 1 , 1984. <br /> B. With respect to non-sworn, "miscellaneous-member" employees, up <br /> to a maximum of twenty-three dollars ($23.00) per month per employee through <br /> June 30, 1984; and up to a maximum of forty-four dollars ($44.00) per month <br /> per employee effective July 1 , 1984. <br /> Any contribution necessary to maintain benefits under any long-term <br /> disability insurance plan provided employees by the City in excess of the <br /> amounts set forth as above shall be borne by the employees. <br /> Section 4. Life Insurance. The City shall maintain in effect for the <br /> term of this Agreement its existing life insurance plans covering employees <br /> covered by the Agreement on the same basis as its plans are offered to <br /> 25 <br />
