additional time, the employee may return to his position provided <br /> that he has a certificate from one of the above mentioned physicians <br /> stating that the employee is able to perform all the duties of <br /> his position without qualification. In addition to the above, <br /> the City Manager may grant a further extension not to exceed a <br /> total of one year without pay. <br /> F. Extension by use of vacation. After an employee' s <br /> sick leave has been exhausted he may be granted permission to <br /> take any earned vacation he may have accrued. <br /> G. Notice. The employee taking sick leave shall notify <br /> his or her immediate supervisor either prior to or within four <br /> ( 4) hours after the time he or she is scheduled to report for <br /> duty. When the absence is more than three (3 ) consecutive working <br /> days the employee must present to his department head a physician' s <br /> certificate stating the cause of illness and that, in the physi- <br /> cian' s opinion , the employee could not report to work because of <br /> such illness or injury and that the employee is sufficiently <br /> recovered to safely return to work. Such certificate shall be <br /> transmitted to the director of personnel with the report of the <br /> return of the employee to work. <br /> A physicians' certificate or other satisfactory written <br /> evidence of actual illness or injury may be required after an <br /> absence of any duration less than three (3 ) days. <br /> H. Denial. No employee shall be entitled to sick <br /> leave with pay while absent from duty because of sickness or <br /> injury purposely self-inflicted or caused by wilful misconduct; <br /> or sickness or disability sustained while engaged in employment <br /> other than employment by the City, for monetary gain or other <br /> compensation, or by reason of engaging in business or activity <br /> for monetary gain or other compensation. <br /> I. Excess Usage. If sick leave is used in excess of <br /> that due and available to an employee, such excess sick leave <br /> will , first, be deducted from any available compensatory time off <br /> benefit; second, from any available vacation leave benefit; finally, <br /> deducted from the next scheduled wage or salary payment. <br /> J. Personal Necessity Leave. Each employee shall be <br /> afforded the opportunity to use up to three (3 ) days (24 hours) <br /> of sick leave benefits per calendar year (on a non-cumulative <br /> basis) for "personal necessity leave" , which circumstances shall <br /> include serious accidents or illnesses to member of the employee' s <br /> immediate family, childbirth, imminent danger to the employee' s <br /> home or other valuable property, or the existence of external <br /> circumstances beyond the employee' s control making it impractical <br /> for him/her to report for duty. <br /> K. Payment for Unused Sick Leave. <br /> (1) Except in cases of disability retirement, upon non- <br /> disciplinary termination of employment after ten ( 10 ) years of <br /> -24- <br />
