provisions of Section A, herein. <br /> 5 . The City shall convert the donated time to <br /> dollars at the hourly base salary rate of the donor. The dollars <br /> shall then be converted to sick leave at the hourly base salary <br /> rate of the recipient of the donation. The appropriate hours of <br /> sick leave will be added to the recipient' s sick leave account for <br /> use during his/her leave of absence without pay due to the <br /> catastrophic medical condition. <br /> ARTICLE XII - EMPLOYEE INSURANCE <br /> Section I . During the term of this Agreement the City will <br /> contribute toward the payment of premiums for health, dental and <br /> long-term disability insurance plans administered by the <br /> Association for the benefit of the employee represented by the <br /> Association as follows : <br /> A. Health Insurance . <br /> Effective November 1, 1996, the City will contribute into <br /> the Santa Ana Police Officer Association Medical Insurance Trust <br /> Fund a maximum of $400 . 00 per month for each eligible employee <br /> enrolled in health insurance plans provided by the Association for <br /> its bargaining unit members and their eligible dependents . Any <br /> contribution necessary to maintain benefits under said health <br /> insurance plan in excess of $400 . 00 per month shall be borne <br /> entirely by the Association and/or the employees . <br /> B . Dental Insurance. The City will contribute into the <br /> Santa Ana Police Officer Association Medical Insurance Trust Fund <br /> a maximum of thirty dollars ($30 . 00) per month for each affected <br /> employee enrolled in dental insurance plans provided by the <br /> Association for its bargaining unit members and their eligible <br /> dependents . Any contributions necessary to maintain benefits under <br /> said dental plans in excess of $30 . 00 per month shall be borne <br /> entirely by the Association and/or the enrollees . <br /> C. Long-Term Disability (L.T.D. ) Insurance . The City <br /> shall continue to contribute to the Santa Ana Police Officer <br /> Medical Insurance Trust Fund an amount toward the payment of <br /> premiums for each employee who is enrolled in the long-term <br /> disability insurance plan provided by the Association for employees <br /> covered by this Agreement in accordance with the following <br /> schedule : <br /> 1 . With respect to sworn, "safety-member" police <br /> personnel, up to a maximum of sixty-eight dollars ($68 . 00) per <br /> month per enrollee . <br /> 2 .
