Set Public Hearing For_ <br />CONTINUED TO <br />FILE NUMBER <br />Authorize the City Manager and Clerk of the Council to execute the attached third amendment to the <br />agreement with Pulmonary Consultants and Primary Care Physicians Medical Group, Inc., subject to <br />non-substantive changes approved by the City Manager and City Attorney, in the amount not to <br />exceed $30,000. <br />DISCUSSION <br />In August 2007 City Council approved an agreement with Pulmonary Consultants and Primary Care <br />Physicians Medical Group, Inc., for annual Hazmat/SWAT physicals. These physicals detect the <br />general health condition of the employee and the increased risk of respiratory and heart disease <br />linked to the exposure of hazardous materials. The Fire and Police departments are requesting <br />amending this agreement by $30,000 for an aggregate amount not to exceed $60,000. <br />FISCAL IMPACT <br />Funds are budgeted in the fiscal year 2010-11 Fire Department, Fire Suppression, Contract Services <br />Professional account, (account no. 01115330-62300, $25,000); and the Police Department, Human <br />Resource Division, Contract Services Professional account (account no. 01114402-62300, $5,000). <br />APPROVED AS TO FUNDS AND ACCOUNTS: <br />David Thomas Francisco Gutierrez <br />Acting Fire Chief Executive Director <br />Fire D?a?tment <br />/ ? <br />?
