E. Any contribution necessary to maintain benefits under any health insurance program <br /> provided by the City for its employees and their eligible dependents in excess of the <br /> amount of the City contribution to the Cafeteria Benefit Plan specified above shall be <br /> borne entirely by the employee. <br /> F. Cafeteria Benefit Plan. The City provides a Cafeteria Benefit Plan adopted in <br /> accordance with the provisions of Internal Revenue Code § 125 for employees <br /> covered by this MOU. <br /> 1. Employees will be eligible to receive through the Cafeteria Benefit flan,the <br /> cash difference(subject to taxation as wages)if the monthly City allowance <br /> exceeds the monthly cost of the Medical and Dental monthly plan premium <br /> cost. <br /> 2. For each such employee who is covered under a group sponsored health plan, <br /> the City will pay the employee a cash-in-lieu payment(subject to taxation as <br /> wages) each month in an amount equal to the City contribution for the <br /> "employee-only" coverage for medical, if the employee waives, in writing, <br /> City-paid coverage.The above specified amount of City contribution towards <br /> such coverage waived will be applied to earned income. If an employee <br /> waives City provided coverage,said employee shall provide proof of medical <br /> insurance coverage in a group-sponsored health plan. Said waiver shall <br /> include a provision warning such employee that reentry into any of the City- <br /> sponsored plans is allowed only at open enrollment, unless there is a <br /> qualifying event, and may require proof of insurability for such employee <br /> and/or family. <br /> 12.2 Dental Insurance, The City agrees to contribute towards the cafeteria Benefit plan the <br /> payment of premiums for dental insurance plans provided by the City for employees and their <br /> eligible dependents on the following basis: <br /> A. One hundred percent(100%) of the premium cost for "employee-only" coverage. <br /> B. The City will contribute up to one hundred thirty dollars ($130) per month per <br /> employee for"family" coverage. <br /> Any contribution necessary to maintain benefits under said dental plans in excess of the <br /> amount of the City contribution to the cafeteria plan specified above shall be borne entirely <br /> by the employee. <br /> For each such employee who is covered under a group sponsored dental plan,the City will <br /> pay the employee a cash-in-lieu payment (subject to taxation as wages) each month in an <br /> amount equal to one-hundred percent (100%) of the City's contribution, if the employee <br /> 38 <br />
