Civic Center Dr. in Santa Ana and met
with DA Investigators Stan Berry and Michael Kendrick. I was told that an Officer Involved Shooting
occurred on the second floor of the structure and that the suspect was taken to OC Global Medical
Center in Santa Ana and pronounced dead.

The parking structure was on the south-west corner of Coach Dick Hill Way and Civic Center Drive. In
the south-east corner of the parking structure on the second floor, I observed the following: Two metal
bars, taser wire, (2) prongs, (1) door, medical paraphernalia, and apparent blood were on the ground in
front of the 8th and 9th stalls over from the east. Santa Ana Police Unit #865 was faced south-east in
front of stalls 8 and 9. A Taser with a cartridge and (1) door were on the ground behind the left side of
unit #865. Santa Ana Police Units #818 and #864 were west of unit #865, and units #821, #831, and
#841 were to the east. Two cartridge cases were on the ground in front of unit #818 which was faced
south-east. The headstamp on both cartridge cases was “S & W WIN 40”. A metal pipe and a metal
wire were leaning against the south rail in front of the 5th stall over from the east.

I proceeded to OC Global Medical Center in Santa Ana and met Deputy Coroner Hoag and DA
Investigator McShane in Trauma Room 1. The decedent was face up on a bed with only one sock on his
right foot and no other clothing on. He had a black latex glove on his right hand and handcuffs around
his right wrist. I observed injuries to his left arm, left armpit and chest.
