EXHIBIT A <br />SCOPE OF SERVICES <br />1) Provider will trach Dance classes at Image Dance Place at 2017 N. Main N Santa Ana. <br />Classes will includeCumbia; learn the basics of Cumbia which is Colombian style Salsa <br />danced South and Central America and Tap/Jazz; Jazz includes a warm-up followed by <br />dance combinations including turns, jumps and leaps. Tap focuses on footwork and sound <br />developing agility, coordination and rhythm. Ballet is a good foundation for any dance <br />form and is great for toning the body. Balleffap and BallefHip Hop are both taught to <br />build rhythmic skills, body awareness, and creativity. Hip Hop Funk focuses on <br />contemporary movements of today. Dance Boot Camp allows for dancers to develop a <br />dancer's awareness of hisiher body Salsa is a very fluid dance style set N Latin music. <br />2) The Cumbia class will be held monthly on Monday, for 14 years and up at 8:30 pm <br />starting in April. TheTap/Jazz class will be held monthly on Mondays, for 5 to 9 years <br />old, at 5:30 pm. The Ballet class will be held monthly on Fridays, for 14 years and up, at <br />6:30 pm. The Balletrfap class will beheld monthly on Saturdays, for 3 to 5 years old, at <br />9:00 am. The BalleVHip Hop class will beheld monthly on Tuesdays, for 3 to 5 years <br />old, at 5:30 pm. The Hip Hop Funk class will be held monthly on Tuesdays, for 14 years <br />and up, at7:30pm. The Dance Boot Camp class will beheld monthly on Fridays, for 14 <br />years and up, at 3:30 pm. The Salsa class will beheld monthly on Thursdays, for 8.14 <br />years old, at 7:45 pm. <br />3) Provider will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of said <br />instruction <br />4) If Provider allows others to reach hismer class, those teachers must be over 21, have <br />obtained and maintain an instructor rating, and be covered by Providers' insurance <br />Provider stall provide City with documentation to verify instructor and insurance <br />requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 1 paid students and no more than a maximum of <br />20. <br />2) No registration will be accepted ager the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second mecting of <br />the class, the class shall be canceled, Provider will be under no obligation to provide <br />services and the City will have no obligations to pay Provider compensation <br />CLASS FEES <br />1) Each participant shall pay a $35,00 class registration fee per monthly session, <br />Anticipated revenue from tis class is $10,000.00. <br />
