S.A. 15-005 <br />Investigator: DEAN FULCHER Approved By: <br />Date of Report: 03/12/15 Date: 03/12/2015 <br />Page 1 of 2 <br /> <br /> <br /> <br />CASE NAME: OSORIO, BENITO <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />TYPE: INITIAL RESPONSE <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />On Wednesday, March 11, 2015, at approximately 1315 hours, Orange County District <br />Attorney (OCDA) Supervising Investigator MARK GUTIERREZ contacted me regarding an officer <br />involved shooting incident involving the Santa Ana Police Department (SAPD). GUTIERREZ <br />requested I respond to 200 South Main Street, in the city of Santa Ana, and assist with the <br />investigation. <br />At 1415 hours, I arrived at the incident location and met with investigators from the <br />OCDA and from the SAPD, who were present to assist in the investigation. <br />At 1513 hours, SAPD Sergeant JEFF SMITH conducted a briefing of the known <br />circumstances surrounding the incident for the personnel involved in the investigation. <br />Following the briefing, GUTIERREZ requested SAPD Detective MATT MCLEOD and I <br />interview potential witnesses to the shooting incident. <br />MCLEOD and I interviewed the following two employees of the Mill Bakery, located at <br />312 South Main Street, who witnessed the shooting incident: <br />• Mill Bakery Employee <br />• Mill Bakery Employee <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br />SUPPLEMENTAL REPORT
