EXHIBIT A SCOPE OF SERVICES 1) Provider will teach a Class on CPR 8L First Aid at Santiago Lawn Bowling Club House. This course is approved by the American Red Cross. Bilingual/Spanish C.P.R. and First Aid classes approved for child care providers, health care providers, nannies, and general public. 2) The classes will be held on Tuesdays, Wednesdays, Saturdays and Sunday, for ages 12 and over from Mon. & Wed. 5:00 — 9:00 p.m., Sat. & Sun. 8:30 a.m. — 4:00 p.m. starting on June 2. Classes will run either 3 or 4 hours per class. Classes will be conducted monthly. 3) Provider will provide and be responsible for equipment, records, personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 4) If Provider allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Providers' insurance. Provider shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE 1) Each class must have a minimum of 10 paid students and no more than a maximum of 20. 2) In the event the minimum number of enrollees is not met by the class meeting the class shall be canceled. Provider will be under no obligation to provide services and the City will have no obligations to pay Provider compensation CLASS FEES 1) Each participant shall pay a $35.00 class registration fee per session for CPR and $35.00 class registration fee per session for First Aid. Anticipated revenue from this class is $10,000.00. A material fee will be collected at the beginning of class and will be paid directly to the instructor. 2) No refunds will be made to participants after class meeting unless the class is cancelled by the City. 3) The City shall collect registration fees from each participant during the registration period. Provider shall not collect fees, but shall refer all interested participants to City for registration.
