EXHIBIT A <br />SCOPE OF SERVICES <br />1) Provider will teach a Class on CPR 8L First Aid at Santiago Lawn Bowling Club <br />House. This course is approved by the American Red Cross. Bilingual/Spanish C.P.R. <br />and First Aid classes approved for child care providers, health care providers, nannies, <br />and general public. <br />2) The classes will be held on Tuesdays, Wednesdays, Saturdays and Sunday, for ages 12 <br />and over from Mon. & Wed. 5:00 — 9:00 p.m., Sat. & Sun. 8:30 a.m. — 4:00 p.m. starting <br />on June 2. Classes will run either 3 or 4 hours per class. Classes will be conducted <br />monthly. <br />3) Provider will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of said <br />instruction. <br />4) If Provider allows others to teach his/her class, those teachers must be over 21, have <br />obtained and maintain an instructor rating, and be covered by Providers' insurance. <br />Provider shall provide City with documentation to verify instructor and insurance <br />requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 10 paid students and no more than a maximum of <br />20. <br />2) In the event the minimum number of enrollees is not met by the class meeting the class <br />shall be canceled. Provider will be under no obligation to provide services and the City <br />will have no obligations to pay Provider compensation <br />CLASS FEES <br />1) Each participant shall pay a $35.00 class registration fee per session for CPR and <br />$35.00 class registration fee per session for First Aid. Anticipated revenue from this <br />class is $10,000.00. A material fee will be collected at the beginning of class and will be <br />paid directly to the instructor. <br />2) No refunds will be made to participants after class meeting unless the class is <br />cancelled by the City. <br />3) The City shall collect registration fees from each participant during the registration <br />period. Provider shall not collect fees, but shall refer all interested participants to City for <br />registration. <br />
