On Friday, 04/08/2023, at approximately 1845 hours, I responded to the area of Shelton and
Myrtle Street, the City of Santa Ana, County of Orange. The call was in regards to an officer
involved shooting that occurred. The suspect was located in the proximality of Shelton and
Myrtle Street. I responded in a marked Santa Ana Police vehicle and wearing a Santa Ana
Police uniform. I was riding in a 2 man patrol vehicle with Corporal N. Lopez.
This is a supplemental report of my actions and observations during this investigation. For
further details, please refer to the initial crime report.
When it was safe to do so during this incident, my department-issued Axon 3 Body-worn
camera (BWC) was activated and recording. This recording was later uploaded into the
SAPD evidence system. Any breaks within the recording are to defer with other officers
about the investigation. All statements made below summarize the facts and circumstances
that shall not be taken as all-encompassing of the event. For the exact dialogue and
sequence of events, please see the BWC footage.
Upon arrival, I observed the suspect laying on the east curbline of Shelton just South of
Myrtle Street. The suspect had multiple gunshot wounds to his extremities and was being
treated by officers on scene.
I was tasked to standby with Officer R. Santaella while I remained in the scene.
I transported Officer Santaella back to the Santa Ana Police Department and waited for
investigators from professional standards and a legal representative to respond. I was
relived by Sergeant T. Johnson at approximately at 2300 hours.
This concluded my actions.
Officer M.
