EXHIBIT A SCOPE OF SERVICES 1) Consultant will teach Introduction to Partner Social Dancing for the City's Leisure Class Program, including: BEGINNING WEST COAST SWING $24.00/4WEEKS Ages 8 yrs 8c Up Fun way and easy to learn. No partner needed. You learn rhythm, balance, turn, and confidence. Class begins October 4. INSTRUCTOR: Diane King LOCATION: Salgado Center, 706 N. Newhope Ave., Santa Ana, (714) 571-4267 DAY TIME Mon 4:00 PM - 5:00 PM Wed 4:00 PM - 5:00 PM BEGINNING SALSA $24.00/4WEEKS Ages 8 yrs .& Up Fun way and easy to learn. No partner needed. Newhope Ave., Santa Ana, (714) 571-4267 DAY TIME Mon 3:00 PM - 4:00 PM Wed 3:00 PM - 4:00 PM 2) The classes will be held on the dates and times agreed between Provider and City 3) Provider will provide and be responsible for equipment, records, and personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 4) If Provider allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Providers' insurance. Provider shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE 1) Each class must have a minimum of 15 paid students and no more than a maximum of 50. 2) No registration will be accepted after the second meeting of class. 3) In the event the minimum number of enrollees is not realized by the second meeting of the class, the class shall be canceled. Provider will be under no obligation to provide services and the City will have no obligations to pay Provider compensation CLASS FEES 1) Each participant shall pay a $24.00 registration fee per month, depending on the class attended.
