SA 12‐022 <br />Investigator: P. Kelly Approved By: <br />Date of Report: 9/10/12 Date:09/10/12 <br />Page 2 of 2 <br /> While at the hospital ALCALA heard ALMANZA tell medical staff he had earlier used <br />methamphetamine.  Medical staff provided ALCALA with a brown paper bag containing clothing <br />which had been removed from ALMANZA. <br />At approximately 0415 hours, RN   contacted  me in the operating waiting <br />room and  provided  me with a copper jacketed bullet core which she said had  been removed <br />from ALMANZA during surgery.  The bullet core was inside of a  clear specimen container with <br />an orange lid.  Affixed  to the exterior of the container I observed  YARD DOE,  on a <br />white label. <br /> At approximately 0510 hours, I received the aforementioned  brown bag from SAPD <br />Officer DANIEL BAEK who had  relieved  ALCALA. <br /> At approximately 0704 hours, I relinquished  the bullet core and  brown bag to Orange <br />County Crime Lab Forensic Specialists RACHAEL KAPP and SUSAN MOORE. <br /> At approximately 0710 hours, KAPP, MOORE and I responded to room 6230 in the <br />Intensive Care Unit, where ALMANZA had been transferred to following surgery.  I observed <br />KAPP and  MOORE take several photographs of ALMANZA under FR#12‐53567. <br />I conducted  the following audio digital recorded  interviews: <br />•   Registered Nurse, Emergency Room <br />•   Medical Doctor, Attending Trauma  Surgeon <br />Refer to the individual interview reports for details.
