S.A. 13-015 <br />Investigator: K DeGraffenreid Approved By: <br />Date of Report: 7-3-13 Date: 07/09/2013 <br />Page 1 of 5 <br /> <br /> <br /> <br />CASE NAME: GONZALEZ, JESSICA <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING- FATAL <br />TYPE: INITIAL RESPONSE/HOSPITAL SCENE <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br /> <br />On Sunday, June 23, 2013, at approximately 0900 hours, Orange County District <br />Attorney (OCDA) Supervising Investigator, MARK GUTIERREZ, contacted me regarding the <br />officer involved shooting of JESSICA GONZALEZ. GUTIERREZ requested I respond to the <br />University of California, Irvine-Medical Center (UCIMC), located 101 The City Drive South, <br />Orange, California, 92868, to handle the hospital scene investigation. <br />At 1010 hours, I arrived at the hospital, and contacted Santa Ana Police Department <br />(SAPD) Officer, JUAN MONTIEL #2569 in the Emergency Room of UCIMC, Douglas Hospital (DH). <br />MONTIEL was seated at the nurses’ station in full view of room DH 12, Bed 16, to secure the <br />scene for investigators. <br />Care Ambulance #299 transported GONZALEZ to UCIMC with MONTIEL following in his <br />SAPD patrol unit. Once at UCIMC, GONZALEZ was transported to Trauma Room A where she <br />received medical care from the following UCIMC medical staff: <br />• UCIMC Trauma Physician <br />• UCIMC Emergency Room Registered Nurse <br />I conducted audio digital recorded interviews of Doctor and Registered <br />Nurse .
