EXHIBIT A <br />SCOPE OF SERVICES <br />1) Instructor shall provide F3elly Dance instruction, including basic belly dance <br />techniques — shimmies, hip drops /lifts, across the floor technique, beginning <br />combinations and veil work. <br />2) The Classes will be held on Wednesday, 1 3 years of age and older at 6 :00 7:00 pm. <br />3) Instructor will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of said <br />instruction. <br />4) If Instructor allows others to teach classes_ those teachers must be over 21, hold a valid <br />instructor rating and be covered by Instructor's insurance. Instructor shall provide City <br />with documentation of compliance with the foregoing. <br />CLASS SIZE <br />I) Each class shall have a minimum number of 5 paid students and no more than 25. <br />2) No registration shall be accepted after the second meeting of the class. <br />3) In the even[ the minimum number of enrollees is not realized by the second meeting of <br />class, the class shall be cancelled. Instructor will be under no obligation to provide <br />services and the City will be under no obligation to pay Instructor compensation. <br />CLASS FEES ��� <br />I) Each participant shall pay $50.00 class registration tee per, week session. <br />Anticipated revenue from this class shall be $10,00.00. Additional fees for pours and <br />uniform purchase shall be through the instructor. <br />2) No refunds shall be made to participants after the tirst week unless class is canceled. <br />3) City shall collect registration fees from participants during the registration period. <br />Instructor shall not collect fees but shall refer interested participants to the City. City <br />shall prepare class rosters and provide copies to the Instructor. Only registered <br />participants shall take part. <br />4) Instructor shall receive seventy percent (70 %) of the total fees collected each month. <br />City shall retain thirty percent (30 %) of the fees collected. <br />6) The City shall be entitled to audit Instructor's records to insure compliance with this <br />Agreement. <br />7) instructor shall not waive tees. <br />
