The methods andlor resources ut~llzed nay include <br /> some ar all of the following: cross-training of staff, in-house staff, electronic case <br /> management systems, ca-enrollments, appointments, e~m.ail and phone call, <br /> 7. ~NFIDE~T~A.L~T~; <br /> The ~ agrees that when any individual applies for or receives services from and <br /> the partner agency thrQUgh the ~ne~top tenter, all infor~riatiQn regarding such <br /> application for or receipt of services shall be confidential inforn~atior~ subject to the <br /> provisions of 4 MFR b1.3S. <br /> The parties agree to honor the confidentialit~r provisions as described in the local ~ <br /> plan, including modifications thereto, and incorporated into this 1VI0 by reference, <br /> Exchanged information shall remain private and canf~dential in accordance with the s <br /> most restrictive cor~f~dentiality requirements of any of the parties collecting, receiving <br /> or sharing information. <br /> S. The Resource Sharing Agreement is meant to be reviewed yearl~r <br /> and maybe n~odx~.ed as needed by any partner as long as it is identified s revision, <br /> and 1t 1s signed and dated bar the agency Administrator. <br /> <br /> 9. D~S~TES, The parties shall first attempt tv resolve all disputes informally. Any <br /> party nay call a meeting of all parties to discuss and resolve disputes. <br /> Should informal resolution efforts fail, the dispute shall be referred to the chair of the <br /> SIB, mho shall place the dispute upon the agenda of its next regular or special <br /> meeting of the Board's executive committee. The Executive omnlittee shall <br /> atterr~pt to mediate and resolve the dispute. <br /> Finally, if the Executive Qittee's resolution efforts fail, any party nay file a <br /> grievance ~n accordance vr~th the state of llfornlaas ~A grievance procedures. <br /> The parties agree to be bound by the final determination resulting from that <br /> procedure. All costs associated 1th the gnevance procedure shall be borne by the <br /> losing part~r. <br /> <br /> 1 SEVERABIL~T ~f any part of this N~~U i found to be null and void, ar is <br /> athervvise stricl~en, the rest of this ~rI~~J shall remain in force. <br /> 3 <br /> <br />
