► Should our field technician not be able trace a specific circuit during the <br />field gathering portion of the study, you will be asked to supply that information, <br />thereby keeping wasted field time to a minimum. <br />► In order to complete the Arc Flash study we require information from your <br />utility company. We will forward you a two page information sheet which we request <br />you either email or fax to your utility representative so they can complete the <br />information. It is absolutely essential that this information is returned to us as soon <br />as possible, as without this the study can not be completed. <br />► In order to maintain costs at a reasonable level Thermotest will supply all <br />the labels and would request that your in house engineering staff be responsible for <br />attaching the labels to the panel fronts. The labels will indicate the exact panel <br />identification and location <br />► Upon completion of all the office work the completed Arc Flash study will <br />be forwarded to you in a PDF format. <br />► An Arc Flash hazard analysis, single line diagram; short circuit study and <br />Arc Flash labels will be performed for as outlined below: <br />Pumping Stations - $2320 per station (7 total) <br />Well Sites - $1160 per site (22 total) minimum of 2 per day <br />NFPA70E Safety Training - $3000 — Maximum of 1 5 people per class <br />The above price holds good until March 31, 201 1. Should this proposal meet with your <br />approval we would require a purchase order so that we can schedule the field work. <br />Please note our payment requirements. We will be billing 50% of the total price once <br />the field gathering information is completed and the remainder will be billed once the <br />final information has been emailed to you.
