N- 2006 - '105 -004 <br />WC required if consultant has <br />employees <br />8 CT 2 7 201U <br />� (o) S APO FOURTH AMENDMENT TO <br />Lori YawY' CONSULTANT AGREEMENT <br />V , TCy n, a O'Len�l> <br />THIS FOURTH AMENDMENT TO CONSULTANT AGREEMENT is entered <br />into on October ' , 2010, by and between LUIS PAUL MARTINEZ ( "Consultant ") <br />and the City of Santa Ana, a charter city and municipal corporation of the State of <br />California ( "City "). <br />RECITALS: <br />A. The parties entered into Agreement N-2006-105, dated October 2, 2006, (hereinaft <br />er <br />"said Agreement ") by which Consultant has provided counseling and instructional <br />services for inmates at the Santa Ana Detention Facility. <br />B. In accordance with the teams and conditions of said Agreement, the parties wish to <br />renew said Agreement for an additional one -year period. <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and <br />subject to all the terms and conditions of said Agreement, as previously amended, except <br />those amended in this Fourth Amendment to Consultant Agreement, the parties agree as <br />follows: <br />1. Section 3, TERM, shall be amended to extend the termination date one year, from <br />June 30, 2010, through June 30, 2011. In order to provide continuous uninterrupted <br />service, all services provided by Consultant since July 1, 2010, shall be included <br />within the Scope of Services of this Agreement. <br />2. Except as herein amended, all terms and conditions of said Agreement, as previously <br />amended, shall remain in full force and effect. <br />IN WITNESS WHEREOF, the parties hereto have executed this Fourth Amendment to <br />Consultant Agreement on the date and year first written above. <br />ATTEST: CITY OF <br />SANTA ANA <br />MARIA D.
