Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Scope of Work: Rehabilitation of 15 single family, owner - occupied homes of low - moderate income families in <br />the City of Santa Ana. Homes are assessed for activities to maintain safe, livable conditions for household <br />members who are unable to provide for themselves either economically and physically. Typical work tasks include <br />organizing contractors and volunteers, obtaining materials and other resources to address improvement to exterior <br />- interior access areas, bathroom - kitchen modifications and including handrails, lighting, grab bars in baths and <br />showers, wheelchair ramps, household appliance - heater repair, electrical - plumbing repair, roofing and painting <br />needs. Rehabilitation modifications and /or repairs enable homeowners to continue living in their Santa Ana homes <br />in safety, warmth and with dignity. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana households to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana households served each quarter.
