City of Santa Ana <br />Scope of Work <br />Name of Organization Community SeniorServ <br />Name of Funded Program Meals On Wheels AND Senior Lunch Program <br />Annual Accomplishment Goal <br />1. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />11,100 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period. <br />960 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Santa Ana CDBG funds will be utilized to help fund the raw food costs of meals on wheels and congregate lunch <br />meals to Santa Ana older adult participants. <br />Participants in the Meals on Wheels program will receive 3 meals per day 5 days per week during the 2010/11 <br />fiscal year. Additionally, clients will receive case management services and other ancillary services when <br />needed (personal care, homemaking, chore, home safety checks, and community referrals). Each client will be <br />assessed quarterly by their assigned case manager/case worker. <br />Participants in the Senior Lunch program's will be provided a daily (Monday-Friday) lunch program at 3 <br />nutrition sites in Santa Ana. <br />Locations: <br />Southwest Senior Center <br />Santa Ana Senior Center <br />Vietnamese Community Center <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
