EXHIBIT A <br />SCOPE OF SERVICES <br />1. Provider shall provide gymnastics lessons which will teach basic gymnastics <br />movements including basic tumbling, body movement, floor exercises, and <br />flexibility training. Each class will include a warm up/stretching component <br />and a cool down component. <br />2. This class meets at Jerome Center Gym or Southwest Senior Center on <br />Tuesdays and Thursdays from 4:30pm-5:30 pm for Beginners and from 5:30- <br />6:30 pm for Intermediate/Advanced students. <br />3. If Provider allows others to teach his/her class, those teachers must be over 21, <br />have obtained and maintain an instructor rating, and be covered by Providers' <br />insurance. Provider shall provide City with documentation to verify instructor <br />and insurance requirements. <br />CLASS SIZE <br />1. Each Gymnastics class must have a minimum of 5 paid students and no more <br />than a maximum of 40 students. <br />2. If the minimum registration is not reached by the second class, the class may <br />be cancelled by mutual agreement of Provider and City, without any <br />compensation owed to Provider. <br />CLASS FEES <br />1. Each class participant shall pay a registration fee of $30/$35 per monthly <br />session. <br />2. No refunds shall be made to participants after the first week of class unless the <br />Parks, Recreation, and Community Services Agency cancels the class. <br />3. The City shall collect the class fees from each participant during the <br />registration period. Provider shall not collect fees, but shall refer all interested <br />participants to City for registration. <br />4. Provider shall retain seventy per cent (70%) of the total fees within fifteen <br />(15) working days after completion of the class session. City shall receive <br />thirty per cent (30%) of the fees collected as an administrative fee. <br />5. Provider agrees that City representative shall be entitled to audit Provider's <br />records to insure compliance with this Agreement. <br />6. Provider may not waive class participation/registration fees. <br />7. City shall prepare class rosters and provide a copy to Provider. Only <br />registered participants may participate in class. <br />
