EXHIBIT A SCOPE OF SERVICES 1. Provider shall provide gymnastics lessons which will teach basic gymnastics movements including basic tumbling, body movement, floor exercises, and flexibility training. Each class will include a warm up/stretching component and a cool down component. 2. This class meets at Jerome Center Gym or Southwest Senior Center on Tuesdays and Thursdays from 4:30pm-5:30 pm for Beginners and from 5:30- 6:30 pm for Intermediate/Advanced students. 3. If Provider allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Providers' insurance. Provider shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE 1. Each Gymnastics class must have a minimum of 5 paid students and no more than a maximum of 40 students. 2. If the minimum registration is not reached by the second class, the class may be cancelled by mutual agreement of Provider and City, without any compensation owed to Provider. CLASS FEES 1. Each class participant shall pay a registration fee of $30/$35 per monthly session. 2. No refunds shall be made to participants after the first week of class unless the Parks, Recreation, and Community Services Agency cancels the class. 3. The City shall collect the class fees from each participant during the registration period. Provider shall not collect fees, but shall refer all interested participants to City for registration. 4. Provider shall retain seventy per cent (70%) of the total fees within fifteen (15) working days after completion of the class session. City shall receive thirty per cent (30%) of the fees collected as an administrative fee. 5. Provider agrees that City representative shall be entitled to audit Provider's records to insure compliance with this Agreement. 6. Provider may not waive class participation/registration fees. 7. City shall prepare class rosters and provide a copy to Provider. Only registered participants may participate in class.
