If SAHA is located in a metropolitan FMR area, the following additional information must be <br />included in the briefing packet in order to receive full points under SEMAP Indicator 7, <br />Expanding Housing Opportunities [24 CFR 985.3(g)]: <br />~ Maps showing areas with housing opportunities outside areas of poverty or minority <br />concentration, both within its jurisdiction and its neighboring jurisdiction <br />~ Information about the characteristics of these areas including job opportunities, schools, <br />transportation, and other services <br />~ An explanation of how portability works, including a list of portability contact persons for <br />neighboring PHAs with names, addresses, and telephone numbers <br />Additional Items to Be Included in the Briefing Packet <br />SAHA will provide the following additional materials in the briefing packet: <br />Information on how to fill out and file a housing discrimination complaint form <br />"Is Fraud Worth It?" (form HUD-1141-OIG), which explains the types of actions a family <br />must avoid and the penalties for program abuse <br />"What You Should Know about EIV," a guide to the Enterprise Income Verification (EIV) <br />system published by HUD as an attachment to Notice PIH 2010-19 <br />5-I.C. FAMILY OBLIGATIONS <br />Obligations of the family are described in the housing choice voucher (HCV) regulations and on <br />the voucher itself. These obligations include responsibilities the family is required to fulfill, as <br />well as prohibited actions. SAHA must inform families of these obligations during the oral <br />briefing, and the same information must be included in the briefing packet. When the family's <br />unit is approved and the HAP contract is executed, the family must meet those obligations in <br />order to continue participating in the program. Violation of any family obligation may result in <br />termination of assistance, as described in Chapter 12. <br />Time Frames for Reporting Changes Required By Family Obligations <br />Unless otherwise noted below, when family obligations require the family to respond to a request <br />or notify SAHA of a change, notifying SAHA of the request or change within 14 days is <br />considered prompt notice. <br />When a family is required to provide notice to SAHA, the notice must be in writing. <br />Family Obligations [24 CFR 982.551] <br />Following is a listing of a participant family's obligations under the HCV program: <br />~ The family must supply any information that SAHA or HUD determines to be necessary, <br />including submission of required evidence of citizenship or eligible immigration status. <br />~ The family must supply any information requested by SAHA or HUD for use in a regularly <br />scheduled reexamination or interim reexamination of family income and composition. <br />~ The family must disclose and verify social security numbers and sign and submit consent <br />forms for obtaining information. <br />iii29iio Page 5-4 <br />
