Success Rate of Voucher Holders <br />Maximum Points: 5 <br />Only applies to PHAs that have received approval to establish success rate payment <br />standard amounts, and isn't effective until the second full PHA fiscal year following the <br />date of HUD approval of success rate payment standard amounts. <br />This indicator shows whether voucher holders were successful in leasing units with <br />voucher assistance. <br />~ Points are based on the percent of families that were issued vouchers, and that became <br />participants in the voucher program. <br />Deconcentration Bonus Indicator <br />Maximum Points: 5 <br />Submission of data for this indicator is mandatory for a PHA using one or more payment <br />standard amount(s) that exceed(s) 100 percent of the published FMR set at the 50 <br />percentile rent, starting with the second full PHA fiscal year following initial use of <br />payment standard amounts based on the FMRs set at the SOrh percentile. <br />Additional points are available to PHAs that have jurisdiction in metropolitan FMR areas <br />and that choose to submit the required data. <br />Points are based on whether the data that is submitted meets the requirements for bonus <br />PART VI: RECORD KEEPING <br />16-VLA. OVERVIEW <br />SAHA will maintain complete and accurate accounts and other records for the program in <br />accordance with HUD requirements, in a manner that permits a speedy and effective audit. All <br />such records must be made available to HUD or the Comptroller General of the United States <br />upon request. <br />In addition, SAHA will ensure that all applicant and participant files are maintained in a way that <br />protects an individual's privacy rights. <br />16-VLB. RECORD RETENTION [24 CFR 982.158] <br />During the term of each assisted lease, and for at least three years thereafter, SAHA will keep: <br />~ A copy of the executed lease; <br />~ The HAP contract; and <br />~ The application from the family. <br />In addition, SAHA must keep the following records for at least three years: <br />~ Records that provide income, racial, ethnic, gender, and disability status data on program <br />applicants and participants; <br />iii29iio Page 16-24 <br />
