XI. RULES FOR PROPOSAL <br />The signer of the proposal must declare in writing that the only person, persons, company <br />or parties interested in the proposal as principals, are named therein; that the proposal is <br />made without collusion with any other person, persons, company or parties submitting a <br />proposal; that it is in all respects fair and in good faith without collusion or fraud, and that <br />the signer of the proposal has full authority to bind the Consultant. <br />A Consultant's proposal for construction management services shall be submitted in two <br />parts: (A) Consultant's Qualifications, and (B) Consultant's Fee and Reimbursable <br />Expenses. Both parts A and B must bear the signature of the individual with the power to <br />bind the proposing firm to the proposal. <br />PART A: Consultant's Qualifications <br />1. Organization and Credentials of the firm. <br />Provide a synopsis of the consulting firm's past experience and qualifications related to <br />this project. <br />2. Proposed staff. <br />Name the resident engineer, or the construction manager, and other staff the firm is <br />proposing, and list their experience, qualifications and education related to this project. <br />3. Understanding the project. <br />Describe the firm's understanding of the tasks of the project and approaches to <br />accomplish them. <br />4. List of construction management projects completed by the Consultant, including all <br />related bridge projects similar to this project as outlined in Section I-GENERAL <br />PROJECT DESCRIPTION of this RFP. <br />List the names and telephone numbers of the owner's representatives of the project and <br />a brief description for each. <br />5. Other relevant Information. <br />PART B: Consultant's Fee and Reimbursable Expenses (Cost Proposal) <br />Submit three copies of cost proposal in a sealed envelope to the Executive Director of the <br />Public Works Agency of the City of Santa Ana. These copies of cost proposal shall not be <br />included in the same envelope containing Part A of the proposal. <br />The cost proposal must include the projected staff hours, hourly rates, unit prices and <br />reimbursable expenses for the project, as follows: <br />8 <br />25E-29
