Project 11 -6744
Request for Approval of Change Order No. 1
May 2, 2012
2. Due to the widening of the intersection of 17th and Bristol Streets, the contract
documents require all the traffic signal poles and equipment to be replaced with
exception of the northeast corner which is not being widened. The signal heads
on that corner are old and deteriorated; therefore Traffic Engineering has
requested that the signal heads on that corner be replaced with new LED signal
heads to match the remainder of the intersection.
Agreed Cost = $6,000
EXTRA WORK AT TIME & MATERIALS
1. During trenching for the proposed 2" irrigation water service on the east
side of Bristol south of 17th Street, a 6' deep utility line was encountered.
This line had been backfilled with concrete; therefore the proposed 2"
service line will have to be installed below the conflicting utility.
Final Cost = $ 3,457
APPROVED AS TO FUNDS AND
ACCOUNTS:
Raul Godinez II, ecutive Director Francisco Gutierrez, Executive Director
Public Works Agency Finance and Management Services Agency ,
K= \construCtion\projects \1'1- 6744 \MEM RFA CCO #02
