Project 1 -6744
Request for Approval of Change Order No. 5
December 4, 207 2
2. The City directed the contractor to construct an additional ,220 SF bus
turn -out on the northeast corner of Bristol and 17th Street. The cost of the
bus turn -out was covered by bid price line items; however, there was no
compensation allowed for traffic control to close one eastbound lane of 7th
street to allow construction of the bus turn -out. This compensates the
contractor for all labor and equipment costs to provide, install, and maintain
traffic control for the duration of the construction of the additional bus turn-
out.
Agreed Price $ 7,240.32
3. Per the direction given in PCO #8 the contractor installed SOOLF of conduit
to make the electrical service connection for the irrigation controller service
pedestal. Southern California Edison requires their own contractor to install
the wiring and make the final connections, thus delaying the completion of
the work. Due to the proximity of Santa Ana College the contractor was
directed to maintain the traffic control and barricading to ensure public
safety. This compensates the contractor for all labor and equipment costs to
provide, install, and maintain the traffic control and barricading until SCE
completed their portion of the work.
Agreed Price $ 9,626.02
4. The contractor was directed in PCO # 9 to import sand, backfill and
compact the unplanted palm tree excavations flush with the adjacent
sidewalk to ensure public safety until the palm trees are planted. This
compensates the contractor for all labor and equipment costs to import
sand, backfill, and compact the excavations.
Agreed Price $ 4,594.24
APPR ED AS TO FUNDS AND ACCOUNTS:
Raul Go Inez I1, Executive rector Francisco Gutierrez, Executive Director
Public W s Agency Finance and Management Services Agency
K:\construction\projects \11- 6744 \MEM RFA CCO #OS
