2011/12 1 2010/11- 2014/15 CITY OF SANTA ANA CONSOLIDATED PLAN <br />ACTION PLAN <br />HOMELESS AND OTHER SPECIAL NEEDS <br />The Action Plan delineates the specific activities that the City will undertake during the <br />2011 -2012 Program Year that address objectives and goals identified in the Consolidated <br />Plan to meet the needs of Santa Ana's homeless and special need populations. <br />As reported in the 2010 -2014 Consolidated Plan, it is estimated there are 4,961 homeless <br />individuals in Santa Ana. To address the needs of these individuals, the City will continue its <br />efforts to identify efficient and effective means to distribute limited resources for homeless <br />needs. The City has designated emergency and transitional housing as high priorities for <br />addressing homelessness. There are 16 emergency and transitional shelters in the City, <br />providing an estimated 265 emergency and 317 transitional housing beds - more than any <br />other city in the County .3 During the 2011 -2012 Program Year the City will also incorporate <br />into its agreements a provision that each ESG- funded agency prepare and implement a <br />policy that will help ensure persons discharged from local publicly funded institution have <br />access to housing /services thus preventing their immediate homelessness upon discharge. <br />During Fiscal Year 2009 -2010, the City implemented the Homelessness Prevention and <br />Rapid Rehousing (HPRP) program. This $2,831,989 HUD grant was authorized under the <br />American Recovery and Reinvestment Act of 2009. The City has allocated these funds to 13 <br />programs managed by nonprofit agencies. HPRP resources will be used to provide Santa <br />Ana residents with homelessness prevention and access to housing permanent over a three <br />year period. Since funding for this program will not continue beyond 2012, <br />accomplishments are not included in the Annual Action Plan. A separate accomplishment <br />report will be completed by the City at the end of each HPRP program year. <br />More specifically, during the 2011 -2012 Program Year, the City will undertake the following <br />homeless housing /service activities: <br />SUPPORTIVE SERVICES <br />Santa Ana's Continuum of Care strategy calls for the provision of support services to assist <br />individuals and families that are homeless or at risk of becoming homeless. ESG resources <br />will be provided to local nonprofit organizations; an estimated 550 individuals will be <br />assisted during FY 2011 -2012, including 500 chronic homeless individuals.
