An estimated 47 individuals will be assisted with <br />transitional housing services during FY 2011 - 2012.5 Specific activities to be carried out are <br />Cielln2dteCl In the LISTING OF PROPOSED PROJECTS— EXHIBIT 3. <br />TRANSITION TO PERMANENT HOUSING <br />Access to decent, affordable permanent housing is the ultimate goal of the City's <br />Continuum of Care system. In the past, the City has provided HOME, CDBG and <br />Redevelopment resources to nonprofit service providers in order to acquire and rehabilitate <br />housing units for persons leaving homeless programs. Some of these units have been <br />targeted for special need populations such as the disabled, youth, and individuals living with <br />HIV /AIDS. During the 2011 -2012 Program Year, the Housing Authority will reserve ten <br />vouchers to assist households graduating out of a transitional housing program. HOPWA <br />funding will be utilized to provide rental assistance to 50 individuals /families living with <br />HIV /AIDS on a countywide basis. <br />SPECIAL NEEDS OF PERSONS NOT HOMELESS BUT IN NEED OF SUPPORTIVE HOUSING <br />Santa Ana will continue to provide federal resources to meet the needs of persons with <br />special needs (e.g., elderly, victims of domestic violence, persons with disabilities, persons <br />living with HIV /AIDS). Various programs will be undertaken during the 2011 -2012 Program <br />Year including housing rehabilitation (which may include installation of items such as ramps, <br />grab bars, wider doors, etc.), to improve mobility for persons with disabilities, and public <br />services.
