2011/12 1 2010/11 - 2014/15 CITY OF SANTA ANA CONSOLIDATED PLAN <br />ACTION PLAN <br />• Address obstacles to meeting underserved needs <br />• Foster and maintain affordable housing <br />• Remove barriers to affordable housing <br />• Evaluate and reduce lead -based paint hazards <br />• Reduce the number of poverty level families <br />• Develop institutional structure <br />• Enhance coordination between public /private housing and service agencies <br />• Foster assisted housing improvements and resident initiatives <br />• Actions taken to address impediments to fair housing choice that may exist in the <br />City <br />• Program Specific Requirements — There are certain program requirements that must be <br />included in the Action Plan for each federal grant program. <br />• Monitoring - A description of the standards and procedures that the City will use to <br />monitor activities carried out in furtherance of the plan and ensure proper use of <br />federal funds. <br />• Certifications - HUD requires the City to submit various certifications related to the <br />implementation of federal grant programs - these are standard HUD certifications. <br />HUD requires that the City implement an outcomes measurement management system. <br />This system is outlined in the 2010 -2014 Consolidated Plan. The City has incorporated <br />outcome measurement requirements into contracts with all sub - grantees and grantee- <br />funded activities. Finally, the City has utilized a portion of HUD's Consolidated Plan <br />Management Program (CPMP) tool to prepare the 2011 -2012 Action Plan. <br />HUD regulations also require that the City prepare an executive summary for each Action <br />Plan that not only summarizes how the City will utilize HUD funds for 2011 -2012, but also <br />reviews prior year accomplishments. Since the 2010 -2011 Fiscal Year was the first year of <br />the 2010 -2014 Consolidated Plan cycle, minimal program /project information from prior <br />years has been collected and reported. In subsequent years, a more complete executive <br />summary will be incorporated into each Annual Plan. <br />The City of Santa Ana is a recipient of four significant grants from the U.S. Department of <br />Housing and Urban Development (HUD); these grants and the amount the City anticipates it <br />will receive are listed below (see TABLE 1): <br />2011 -2012 ANNUAL ACTION PLAN 2 04/21/2011 <br />Exhibit 1 <br />
