EXHIBIT A <br />SCOPE OF SERVICES <br />1) Consultant will teach a Boot Camp class at Santiago Park. <br />FUNctional TRAINING $120.00/SESSION <br />Ages 18 yrs &t Up $60.00/SESSION <br />Make exercise fun again by joining a boot camp run by CNI College's Personal Training <br />Department. Come experience a fun, energetic, and varied workout in a boot camp setting. These <br />workouts promote weight loss, boost your sports performance, and increase your daily energy. <br />The boot camp offers Pre and Post fitness assessments. <br />INSTRUCTOR: LaKisha Gaither <br />LOCATION: Santiago Park, 600 E_ Memory Lane., Santa Ana (714) 647-5307 <br />DAY TIME <br />Mon -Thu 8:30 AM - 9:30 AM <br />Mon -Thu 1:00 PM - 2:00 PM <br />Mon -Thu 6:30 PM - 7:30 PM <br />Sat, Sun 10:00 AM - 1 1:00 AM <br />2) The classes will be held on Monday, Tuesday, Wednesday, Thursday, Saturday and Sunday at <br />Santiago Park. <br />3) Consultant will provide and be responsible for equipment, records, and personnel and clean up <br />of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. <br />4) If Consultant allows others to teach his/her class, those teachers must be over 21, have <br />obtained and maintain an instructor rating, and be covered by Consultants' insurance. Consultant <br />shall provide City with documentation to verify instructor and insurance requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 2 paid students and no more than a maximum of 13. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of the <br />class, the class shall be canceled. Consultant will be under no obligation to provide services and <br />the City will have no obligations to pay Consultant compensation <br />CLASS FEES <br />1) Each participant shall pay a $120.00/$60.00 registration fee per month, depending on the class <br />attended. Anticipated revenue from this class is $7,200.00. <br />N <br />
