INSURANCE NOT ON FILE <br />WORK MAY NOT PROCEED <br />CLERK OF COUNCIL <br />DATE: ,IUN 1 3 2011 <br />G 1P6X,C� (`% <br />LAY � f �\lJY1 <br />K OSx.) <br />SECOND AMENDMENT TO AGREEMENT <br />N-2009-057-002 <br />THIS SECOND AMENDMENT TO AGREEMENT is entered into this it day <br />of, " 1` , 2011, by and between Community Veterinary Hospital, <br />Inc., a California corporation ("Consultant") and the City of Santa Ana, a charter city and <br />municipal corporation of the State of California ("City"). <br />Recitals: <br />A. The parties entered into Agreement #N-2009-057, dated May 20, 2009, (hereinafter <br />"said Agreement") by which Consultant has provided veterinary medicine and <br />services for Police Service Animals. <br />B. The parties entered into First Amendment to Agreement #N-2009-057-001, dated <br />June 10, 2010, to renew the term of said Agreement for an additional one-year period <br />and update the scope of services and rates for Consultant's services. <br />C. In accordance with the terms and conditions of said Agreement, the parties wish to <br />renew the term of said Agreement for an additional one-year period and increase <br />compensation to provide for services during the extended term. <br />Wherefore, in consideration of the covenants contained in said Agreement, and subject <br />to all the terms and conditions of the Agreement, except those amended in this Second <br />Amendment to Agreement, the parties agree as follows: <br />1. Section 2, COMPENSATION, subsection a, shall be amended to increase <br />compensation to pay for services during the extended term. The total sum to be <br />expended under this Agreement shall not exceed $25,000.00, annually, during the <br />amended term of this Agreement. <br />3. Section 3, TERM, shall be amended to extend the term for an additional one-year <br />period, from July 1, 2011 through June 30, 2012. <br />4. Except as hereinabove modified, all terms and conditions of said Agreement shall <br />remain in full force and effect. <br />
