A description of: <br />• Overall qualifications of the Proposer and experience and ability to provide NEC <br />Telephone System maintenance and/or CENTIGRAM Voicemail System maintenance <br />services. <br />• The firm's certifications/authorizations to provide services on NEC and/or CENTIGRAM <br />systems/equipment as identified in section 4.0. Also provide evidence of such as part <br />of the proposal. <br />• Any unique qualifications and experience which distinguish Proposer's ability to provide <br />NEC Telephone System and/or CENTIGRAM Voicemail System maintenance services. <br />e. A confirmation of the Proposer's willingness and capability to agree to and to follow the <br />terms and processes outlined in this RFP and resulting agreement including all <br />administrative and billing procedures of the City. <br />Any other information the Proposer feels is relevant in helping the City gauge the <br />Proposer's business stability. <br />• Identify whether the Proposer had a contract terminated for cause within the last 3 <br />years. If so, include an explanation of the circumstances. <br />• Identify whether either a customer or a vendor sued the Proposer in the last 3 years. <br />If so, include an explanation of the circumstances. <br />• Identify the percentage of Proposer's business consisting of servicing and supporting <br />NEC Telephone Systems and/or CENTIGRAM Voicemail Systems. <br />g. A description of: <br />• The qualifications, experience and abilities to provide NEC Telephone System and/or <br />CENTIGRAM Voicemail System maintenance services of the Proposer's candidate on- <br />site technician, backup technicians, and any other engineering and service staff as <br />applicable. <br />• The certifications on NEC and/or CENTIGRAM systems/equipment (as identified in <br />section 4.0) of the Proposer's candidate on-site technician, backup technicians, and <br />any other engineering and service staff as applicable. <br />¦ Evidence of certifications as part of the proposal. <br />• Any unique qualifications and experience as applicable which distinguish Proposer's <br />candidate on-site technician's, backup technicians', and any other engineering and <br />service staff's ability to provide NEC Telephone System and/or CENTIGRAM Voicemail <br />System maintenance services. <br />• For NEC Telephone System maintenance proposer only: Proposer's candidate on-site <br />technician's and backup technicians' working knowledge of, skills in, and experience <br />with Category 5 cabling and termination in accordance with the Electronic Industries <br />Association/Telecommunications Industry Association (EIA/TIA) standard practices. <br />¦ Provide evidence of any related training as part of the proposal. <br />h. Provide a response to the list of services sought by the City by completing the table titled <br />"5.2 General Services & Support" located in the Proposal Response Form, Section 2. <br />Services Proposal [see Appendix A]. <br />ii <br />25A-21
