Respond to each of the following services sought by the City and related terms and conditions as the <br />Proposer's willingness and ability to provide or comply. Yes Li No <br />for the City's CENTIGRAM Voicemail System <br />The Vendor shall: <br />• Provide all required maintenance (parts and labor) and ? Yes Li No <br />maintain the CENTIGRAM in good operating condition in <br />accordance with the manufacturer's technical <br />specifications. This is to include the most current software <br />release for all telecommunications equipment at no <br />additional charge to the City. <br />• Use only original equipment manufacturer (OEM) parts for ? Yes Li No <br />repair or replacement of defective equipment. <br />• Perform preventive maintenance on the CENTIGRAM in ? Yes Li No <br />accordance with manufacturer's specifications. <br />¦ Preventive maintenance shall be performed on a Li Yes Li No <br />schedule which is workable with the City's operating <br />requirements and which is mutually acceptable to the <br />City and the Vendor. <br />¦ Completed preventive maintenance checklists will be ? Yes Li No <br />provided to the City Telecommunications Coordinator <br />on a monthly basis. <br />• Perform monthly system backups and an Offline Li Yes ? No <br />Run/Verify every six (6) months. <br />• Maintain an adequate on-site inventory of spare OEM ? Yes ? No <br />parts for repairs and replacements. <br />¦ The Vendor will identify the amount and type of L3 Yes Li No <br />storage space it prefers. Actual storage space to be <br />provided by City will be determined by the City <br />Telecommunications Coordinator. <br />• Respond within four hours (excluding weekends and Li Yes Li No <br />holidays) of Vendor's receipt of the City's service request <br />when Minor Repairs are necessary during normal City <br />working hours (8-5). <br />¦ Minor Repairs should be completed within twenty- ? No <br />four (24) hours. <br />• Respond within two (2) hours of the City's notification by u Yes Li No <br />the Telecommunications Coordinator or his/her designee <br />in case of Major System Failure during normal City <br />working hours (8-5), <br />¦ In case of Major System Failure, before or after Li Yes ?
