EXHIBIT A <br />SCOPE OF SERVICES -- Maria Ruiz/Pre school <br />A. Instructor shall conduct Pre School Program classes for children ages 3.5 -Syrs old. <br />The program will help child grow to be independent, able to work well with others <br />and gain teamwork skills. <br />Location: Corbin Center 2215 W. Mc Fadden Ave., Santa Ana, CA. 92704 <br />B. Classes shall be 4- hours, five days a week, conducted in one -month sessions during <br />the months of June through August and September through December November and <br />December shall be three -week sessions. <br />C. Instructor shall work with City staff in setting a schedule for classes, including the <br />location, days and hours when class will be held and holidays to be observed. <br />D. Instructor shall provide materials, supplies, equipment, records and personnel. <br />Instructor shall be responsible for clean -up of the facilities and materials and shall <br />ensure the safety and effectiveness of instruction. <br />CLASS SIZE - REGISTRATION <br />A. The minimum number of registered and paid participants is 18 children per class. <br />The maximum is 25. <br />B. In the event the minimum number of enrollees is not realized by the first class, the <br />class may be cancelled by mutual agreement of Instructor and City. In such event, no <br />compensation shall be owed Instructor. <br />FEES <br />A. The class fee is $25.00 per week per participant. $5.00 material /supplies are collected <br />on the first day of class and payable directly to the Instructor. <br />B. No ref�uids shall be made to participants unless the class is cancelled as set forth in <br />Section II.B, above. City shall register and collect fees from each participant in the <br />class during the period of registration. City shall pay Instructor eighty percent (80 %) <br />of the total fees collected each month. City shall retain twenty percent (20 %) of the <br />fees collected as an administrative fee. Total payments provided to Instructor shall <br />not exceed $25,000.00 per fiscal year. <br />C. City shall be entitled to audit Consultant's records to ensure compliance with this <br />Agreement. <br />D. Only registered and paid participants may participate in class. <br />0 <br />
