I- �shibit ;� <br />SCOPE OF SERVICES— Suzanne Ryan% Haercise- [lealth� vloves, Healrh� Lies <br />1) City will advertise your classes as two days a week, 1 hour of class conducted in 4 <br />week sessions during the month of October, and November and December shall be three - <br />week sessions. Brochure covers January through March (Winter), April through May <br />(Spring), and June through August (Summer). <br />Exercise— Healthy Moves, Healthy Lives $12 /Month <br />Ages 45 yrs &c 99 <br />An exercise program developed for any age specifically, with an emphasis on older adults. The class has <br />been structured to focus on four key areas important to the health and fitness of mature participants: <br />stretching and flexibility; low impact aerobics; strength training; and balance. A fun atmosphere in a level <br />within your capacity. Each session, provides social stimulation as well as physical benefits. Class begins <br />Oct 4. <br />INSTRUCTOR: Suzanne Ryan <br />LOCATION: Southwest Senior Center 2201 W. Mc Fadden Ave., Santa Ana, (714) 647 -5306 <br />DAY TIME <br />Tue & Thurs 10:30 PM - 11:30 PM <br />2) Consultant will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of said <br />instruction. <br />3) If Consultant allows others to teach his/her class, those teachers must be over 21 , have <br />obtained and maintain an instructor rating, and be covered by Consultants' insurance. <br />Consultant shall provide City with documentation to verify instructor and insurance <br />requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 3 paid students and no more than a maximum of <br />50. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of <br />the class, the class shall be canceled. Consultant will be under no obligation to provide <br />services and the City will have no obligations to pay Consultant compensation <br />CLASS FEES <br />1) Each participant shall pay a $12.00 class registration fee per month. Anticipated <br />revenue not to exceed $25,000. <br />2) No refunds will be made to participants after the first week of class unless the class is <br />cancelled by the City. <br />3) The City shall collect registration fees from each participant during the registration <br />period. Consultant shall not collect fees, but shall refer all interested participants to City <br />for registration. <br />
