Section 5 <br />General Health and Safety Requirements <br />5.1 Tailgate Safety Meetings <br />All personnel assigned to perform work described in this HASP must be (1) given a <br />personal copy of this HASP; (2) briefed on the health and safety requirements of this <br />HASP by a CONTRACTOR Health and Safety Manager; and (3) acknowledge receipt <br />and willingness to comply with provisions of the plan by signing the Employee <br />Acknowledgement form in Appendix A. Individuals refusing to sign the agreement <br />will not be permitted to conduct field work for this project. Completed agreements <br />will be provided to the CONTRACTOR Project Manager, who will file them with the <br />CONTRACTOR Health and Safety Manager. A tailgate safety meeting will be <br />conducted prior to commencing drainage inspection and cleaning activities. <br />5.2 Incident Reporting <br />All health and safety incidents shall be reported to City of Santa Ana Project Manager, <br />CONTRACTOR Project Manager, and CONTRACTOR Health and Safety Manager <br />immediately. The prompt investigation and reporting of incidents will reduce the <br />risk of future incidents, better protect all field crews. <br />Incident reports shall be prepared by completing an Injury/ Illness Report. This form <br />may be obtained from the CONTRACTOR Project Manager and is located in <br />Appendix A. <br />5-1 <br />19E-28
