Prepared training <br />materials for staff on case documentation, established program <br />evaluation criteria, and assisted with program monitoring activities. <br />1977 Hillhaven Skilled Nursing Facility <br />Social Work Internship <br />Conducted multidisciplinary patient care conferences to review patient <br />needs.
