CITY OF SANTA ANA <br />PROPOSITION 84 <br />SECTION 75022 <br />LETTER OF COMMITMENT ATTACHMENT <br />The California Department of Public Health (State) has determined that the project <br />proposed by the City of Santa Ana (Supplier), Project No. P84C-3000663-003C, is <br />eligible for a grant in the amount of $1,121,727. The project is for the consolidation of <br />the Diamond Park Mutual Water Company with the City of Santa Ana and includes the <br />construction of a water distribution system, installation of laterals and meters to connect <br />each property to the Supplier, and destruction of Diamond Park Mutual Water <br />Company's only groundwater well (Project). Once the project is completed, the <br />Diamond Park Mutual Water Company (Public Water System No. 3000663) will no <br />longer exist as a public water system. <br />Funding is contingent upon the Supplier's compliance with the following terms and <br />conditions. Compliance shall be determined at the sole discretion of the State or its <br />authorized representative. <br />Not later than six (6) months following the date Supplier signs this Letter of <br />Commitment (LOC), unless otherwise specified in this LOC, Supplier shall <br />complete all requirements listed in Section I before a funding agreement <br />will be issued for signature by the Supplier's authorized representative. <br />Supplier shall submit to the State a written resolution, duly adopted by <br />Supplier's governing body, identifying individual(s) with legal authority to: <br />a. sign the funding agreement; <br />b. approve claims for reimbursement; <br />c. sign the Budget and Expenditure Summary; <br />d. sign the Final Release form; and <br />e. sign the certification that the project is complete and ready for final <br />inspection. <br />Person(s) signing the Budget and Expenditure Summary and certification <br />that the project is complete and ready for final inspection must be a <br />registered engineer(s) or person(s) approved by CDPH. <br />2. Supplier shall submit to the State a written designation of the Supplier's <br />Grant Administrator. <br />Note: You are encouraged to submit a draft of the statements, resolutions, or <br />ordinances to the State for review prior to taking it to your board or <br />governing body for adoption or approval. State will review the draft and <br />recommend to you any required modifications, thus avoiding unnecessary <br />delays in issuance of your funding agreement.
