EXHIBIT A <br />SCOPE OF SERVICES <br />1) Instructor will teach dance classes throughout the year as follows: <br />LITTLE BALLERINAS (ages 3 '/1 -5 yrs & 6 — 12 yrs) $25.00 per four-week session <br />ZUMBA (ages 14 & up) $25.00 per four-week session or $9.00 daily <br />2) The classes will be held on the dates and times agreed between Instructor and City. <br />3) Instructor will provide and be responsible for equipment, records, and personnel and <br />clean up of the facilities and materials necessary to ensure the safety and effectiveness of <br />said instruction. <br />4) if Instructor allows others to teach his/her class, those teachers must be over 21, have <br />obtained and maintain an instructor rating, and be covered by Instructors' insurance. <br />Instructor shall provide City with documentation to verify instructor and insurance <br />requirements. <br />CLASS SIZE <br />1) Each class must have a minimum of 5 paid students and a maximum of 20 for Little <br />Ballerinas, 65 for Zumba. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of the <br />class, the class shall be canceled. Instructor will be under no obligation to provide services <br />and the City will have no obligations to pay Instructor compensation <br />CLASS FEES <br />1) Each participant shall pay the class registration fee. <br />2) No refunds will be made to participants after the first week of class unless the class is <br />cancelled by the City. <br />3) The City shall collect registration fees from each participant during the registration period. <br />Instructor shall not collect fees, but shall refer all interested participants to City for <br />registration. <br />4) Instructor shall receive seventy per cent (70%) of the total fees collected each month. City <br />and Instructor agree that City shall retain thirty per cent (30%) of the fees collected as an <br />administration fee. <br />5) Instructor agrees that City is entitled to audit Instructor's records and classes to insure <br />compliance with this Agreement. <br />6) Instructor may not waive class participation/registration fees. <br />7) City shall prepare class rosters and provide a copy to Instructor. Only registered participants <br />may participate in class <br />6 <br />
