Such listing shall also include the full names <br /> <br /> 2 of all of CONTRACTOR's officers, directors, partners and those holding a <br /> 3 financial interest. Included are members of its advisory boards, members of <br /> <br /> 4 its staff and consultants, who have any family relationship by marriage or <br /> 5 blood to an officer, director, or stockholder of the corporation or to any <br /> <br /> 6 partner of the partnership. In preparing the latter listing, CONTRACTOR shall <br /> 7 also indicate the names of the officers, directors, stockholders, or <br /> <br /> 8 partner(s), as appropriate, and the family relationship which exists between <br /> i <br /> 9 such person(s) and CONTRACTOR's representatives listed. <br /> 10 8.3.5 True and correct copies of all agreements with respect to <br /> <br /> 11 any such real property shall be appended to the affidavit described above and <br /> 12 made a part thereof. If, during the term of this Agreement, there is a change <br /> <br /> 13 in the agreement(s) with respect to real property where persons receive <br /> 14 services, CONTRACTOR shall promptly notify ADMINISTRATOR, in writing, <br /> <br /> 15 describing such changes. <br /> 16 9. NON-DISCRIMINATION <br /> <br /> 17 9.1 In the performance of this Agreement, CONTRACTOR agrees that it <br /> 18 shall not engage nor employ any unlawful discriminatory practices in the <br /> <br /> 19 admission of clients, provision of services or benefits, assignment of <br /> 20 accommodations, treatment, evaluation, employment of personnel or in any other <br /> <br /> 21 respect on the basis of sex, race, color, ethnicity, national origin, <br /> 22 ancestry, religion, age, marital status, medical condition, sexual <br /> <br /> 23 orientation, sexual preference, physical or mental disability or any other <br /> 24 protected group in accordance with the requirements of all applicable Federal <br /> <br /> 25 or State laws. <br /> 26 9.2 CONTRACTOR shall develop an Affirmative Action Program Plan which <br /> <br /> 27 meets the lawful and applicable requirements of the U.S. Department of Health <br /> 28 and Human Services. <br /> <br /> <br /> (WAM0811) 13 of 40 (December 22, 2011) <br /> <br /> 20B-15 <br />
