Exhibit A SCOPE OF SERVICES-OCCTAC 1) Consultant will teach a variety of Classes in Music, Dance, Art, etc., for the City's leisure class program, including the following: CREATIVE DANCE (Ages 3-4 years; $35/month) PRE -BALLET (Ages 5-6 years; $35/month) MUSIC THERAPY (Ages 3 years and up; $40/month) PIANO LITTLE MOZART 1.0 (Ages 5-6 years; $35/month) GUITAR 1.0 (Ages 7-11 years; $40/month) LATIN AMERICAN HARP (Ages 7 years & up; $35/month) CREATIVE ART (Ages 4-6 years; $35/month) PAINTING & DRAWING (Ages 4-12 years; $35/month) 2) The classes will be held on various days and times at the Orange County Children's Therapeutic Arts Center. 3) Consultant will provide and be responsible for equipment, records, personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 4) If Consultant allows others to teach his/her class, those teachers must be over 21, have obtained and maintain an instructor rating, and be covered by Consultants' insurance. Consultant shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE 1) Class size will vary depending on the type of class. 2) No registration will be accepted after the second meeting of class. 3) In the event the minimum number of enrollees is not realized by the second meeting of the class, the class shall be canceled. Consultant will be under no obligation to provide services and the City will have no obligations to pay Consultant compensation CLASS FEES 1) No refunds will be made to participants after the first week of class unless the class is cancelled by the City. 2) The City shall collect registration fees, at the levels set forth above, from each participant during the registration period. Consultant shall not collect fees, but shall refer all interested participants to City for registration. 3) Consultant shall receive seventy per cent (701/o) of the total fees collected each month. City and Consultant agree that City shall retain thirty per cent (30%) of the fees collected as an administration fee.
