INSURANCE N irf ON PILE A-2011-214 <br />WORK MAY lQ7 PRTEED <br />OCT 07 2011 <br />` AMENDMENT TO CDBG PUBLIC SERVICES <br />MEMORANDUM OF UNDERSTANDING <br />(City of Santa Ana/Parks, Recreation & Community Services Agency) <br />THIS AMENDMENT TO CDBG PUBLIC SERVICES MEMORANDUM OF <br />UNDERSTANDING ("MOU") is made as of July 1, 2010, by and between the City of Santa Ana <br />("City") and the Parks, Recreation and Community Services Agency ("PRCSA"). <br />RECITALS <br />A. City and PRCSA entered into that CDBG Public Services Memorandum of <br />Understanding (MOU) dated July 1, 2011 awarding PRCSA with CDBG funds in the amount of $60,000 <br />to support the Senior Meals Programs (the Program) more fully described in Exhibits A and B to the <br />MOU. <br />B. The parties hereto now desire to amend the MOU to increase funding and to enter into a <br />subrecipient agreement with Community SeniorServ to manage the Program. <br />NOW, THEREFORE, in consideration of performance by the parties of the promises, covenants <br />and conditions herein contained, the parties hereto agree as follows: <br />1. Exhibits A and B to the MOU shall be replaced with the Subrecipient Agreement with <br />Community SeniorServ to manage and perform the Senior Meals Programs consisting of the Meals on <br />Wheels Program and the Senior Lunch Program (said Agreement is attached hereto and incorporated <br />herein as Exhibit 1). <br />IN WITNESS WHEREOF, the parties have executed this Amendment to Memorandum of <br />Understanding as of the date first written above. <br />ATTEST: <br />By: L P ?
