Raul Godinez II
To: Executive Director, PWA
Tyrone Chesanek
;From: Acting Principal Civil Engineer
L1 Y
F
Y
„tW F
Date: May 31, 2012
REQUEST FOR PRIOR APPROVAL OF CHANGE ORDER
Subject: PROJECT NOS. 11 -2605 GOLDEN LOOP BIKE TRAIL REHABILITATION &
11 -2612 MAPLE STREET BIKE TRAIL ENHANCEMENT
EXTRA WORK AT AGREED PRICES
At the Maple Street Bike Trail rest area the contract documents show plugs of native
plants as part of the landscaping plan. These plugs are seasonal are not available this
time of year; therefore Design Engineering has requested that 1 gallon plants be planted
instead. (PCO 3)
ESTIMATED COST = $ 1500
At the Maple Street Bike Trail rest area the contractor installed a new double check
backflow device per the contract documents. Double check backflow devices are not
permitted for irrigation use per S.A.M.C. Sec. 39 -28; therefore the device needs to be
removed and replaced with a lead -free reduced pressure principle device. (PCO 4)
AGREED COST = $ 1230
On the east side of the Maple Street Bike Trail rest area the project abuts private
property. Most of the properties have a block wall adjacent to the rest area that provides
security and screening of the property from users of the trail. Two of the properties have
makeshift fences that offer little screening and security from the trail users. Design
Engineering has requested that a chain link fence be installed and vines planted along the
property line at these locations. (PCO 5)
ESTIMATED COST = $ 5000
Contract Amount $ 614,859
Previous CCOs $ 15,772
This Request $ 7,730
Total CCOs $ 23,502
% of Contract Amount 3.82%
