4 <br />COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM <br />CITY OF SANTA ANA <br />CITY AGENCY PROPOSAL <br />FISCAL YEAR 2012-13 PROGRAM <br />AGENCY: Parks, Recreation, and Community Services Agency <br />PROJECT NAME: Senior Meals <br />AMOUNT REQUESTED: $ 80,000 <br />The program will provide the following. <br />Congregate Meals: <br />Participants in the Senior Lunch Program will be provided a daily (Monday-Friday) hot nutritional lunch at <br />3 nutrition sites in Santa Ana. <br />Locations: <br />Southwest Senior Center <br />Santa Ana Senior Center <br />Vietnamese Community Center <br />Participants will also be part of nutrition education on various senior health topics every quarter by our <br />Dietician. <br />Home Delivered Meals: <br />Participants in the Home Delivered Meals program will receive 3 meals per day 5 days per week and case <br />management services. Meals will be delivered by paid and volunteer drivers. Eligibility and case <br />management services will be provided by CSS Case Managers. Participants will also have access to other <br />services at no charge including in-home services, respite services, home safety equipment, and other <br />resources. <br />75A-16
