I B I :Total number of households receiving transitional/short-term housing <br />assistance whose tenure exceeded 24 months <br />Section 2. Prevention of Homelessness: Assessment of Client Outcomes on Reduced Risks of Homelessness <br />(Short-Term Housing Subsidy Assistance) <br />Report the total number of households that received STRMU assistance in Column [1]. <br />In Column [2], identify the outcomes of the households reported in Column [1] either at the time that they were known to have <br />left the STRMU program or through the project sponsor or subrecipient's best assessment for stability at the end of the operating <br />year. <br />Information in Column [3] provides a description of housing outcomes; therefore, data is not required. <br />At the bottom of the chart: <br />• In Row la., report those households that received STRMU assistance during the operating year of this report, and the <br />prior operating year. <br />• In Row lb., report those households that received STRMU assistance during the operating year of this report, and the <br />two prior operating years. <br />Data Check: The total households reported as served with STRt111U in Column [1] equals the total reported in Part 3, Chart 1, <br />Rou, 4, Column b. <br />Data Check: The stmt of Cohann [2] should equal the number of households repotted in Column [I]. <br />Assessment of Households that Received STRMU Assistance <br />[1] Output: Total [2] Assessment of Housing Status 131 HOPWA Client Outcomes <br />number of <br />households <br /> Maintain Private Housing without subsidy <br /> (e.g. Assistance provided/completed and client is stable. not <br /> likely to seek additional support) <br /> Other Private Housing without subsidy <br /> (e.g. client switched housing units and is now stable, not likely <br /> to seek additional support) Stable/Permanent Housing (PH) <br /> <br /> Other HOPWA Housing Subsidy Assistance <br /> Other Housing Subsidy (PH) <br /> Institution <br /> (e.g. residential and long-teen care) <br /> Likely that additional STRMU is needed to maintain current <br /> housing arrangements <br /> Transitional Facilities/Short-term Temporarily Stable, lvith <br /> (e.g. temporary or transitional a•rangeniea) Reduced Risk of Homelessness <br /> Temporary/Non-Permanent Housing arrangement <br /> (e.g. gave up lease, and moved in with family orftiends but <br /> expects to lire there less than 90 days) <br /> Emergency Shelter/street <br /> Jail/Prison Unstable Arrangements <br /> Disconnected <br /> Death Life Event <br />I a.
